Our Community Survey

We want to hear what YOU want and need from a Birth Center in your south side community. Your answers are confidential and will be used to assess the need, drive the service provision, and program development of Chicago South Side Birth Center. We also aim to share these responses (strictly anonymously) with community members and partners organizations. This survey will take 5-7 minutes to complete. You do not have to be currently pregnant or planning pregnancy to take this survey. We do ask that you be a south side resident, and someone interested in seeing increased healthcare access in your community.
